To apply for the SAMNet Vets program, here’s what you need to do. Note that incomplete documents will be discarded.
Complete and submit the online SAMNet Vets application form, taking care to fill in all fields. This form will automatically create a SAMNet account which is active for 30 days, pending the submission of the signed paperwork. If a Serotek account for the veteran already exists, please enter either user name or account number in the appropriate fields on the form. If you believe you may already have a Serotek account but aren’t sure, you may call Serotek customer service at (650) 249-1000 and a representative will be happy to assist you.
Once the form has been submitted, an email will be sent to the VIST professional listed on the form, in order to verify eligibility. The VIST professional should print the form using the link found in the email. The form should be faxed to 612-392-3881, or emailed as a PDF document to info@serotek.com. Please include a cover letter on your organization’s letterhead stating that the enclosed documents are to be used for participation in the SAMNet Vets program. The documents will be examined upon receipt and if all is in order, the SAMNet account for the veteran will become active for life.
